Privacy Policy

Privacy and Sharing of Information

I authorize the clinic and its associated health professionals to collect my personal and medical information as documented above. In addition, I authorize the clinic and its associated health professionals to communicate with my family doctor and/or referring doctor as deemed necessary for my beneficial treatment. I also understand that my personal and medical information is confidential and will only be disclosed to third parties with my permission.

 

 Counseling Consent and Confidentiality

Benefits and Risks There are anticipated benefits and risks that are associated with receiving my counseling services. Some of the benefits of counselling at Two Feather Wellness include, but are not limited to, learning and developing new skills, developing self-awareness, gaining personal empowerment, and gaining insight into problems that are affecting your life. Risks include but are not limited to: feeling strong, uncomfortable emotions, bringing up difficult memories, and increased feelings of overwhelm and/or distress. In addition, changes may occur during your therapy, and these changes may impact the way you relate to yourself and others.

Confidentiality

We are committed to protecting your privacy. All counseling services provided by Two Feather Wellness are strictly confidential, including your personal information. The release of any information regarding your involvement with Two Feather Wellness may occur only with your consent. However, there are exceptions where the counsellor will share information with the proper authorities.

Exceptions include:

  • If the counsellor has reason to believe that a child or elder needs protection, and/or is at risk for harm and/or neglect;

  • There is reason to believe that you are likely to cause serious physical harm to yourself or others;

  • If the counsellor is required by court order to disclose specific records or to attend court & give evidence (e.g., case notes), and/or

  • When the counsellor has a consultation with a clinical supervisor.

  • Your counsellor will make a reasonable effort to discuss these circumstances with you before involving other professionals.

Collection and Storage of Personal Information

Storage and collection of client information is in accordance with the Personal Information Protection Act (“PIPA”) and with the British Columbia College of Social Workers (“BCCSW”) guidelines. Please do not hesitate to ask me if you have any questions or concerns. For telehealth, online/video sessions are in accordance with Health Insurance Portability and Accountability Act (“HIPAA”) regulations and protect clients’ personal information.

Video sessions will be held on Zoom or through the Jane App’s built-in Video platform. Any text messages, voicemails, and calls will be held on a password-protected cellular device used by only the practitioner themselves. Two Feather Wellness will use all reasonable means to protect the security and confidentiality of the information sent and received in the video appointment. However, because of the risks outlined below inherent to all electronic communications, the security and confidentiality of electronic communications cannot be guaranteed, as follows:

  • Use of electronic communications to discuss sensitive information can increase the risk of inadvertent disclosure of such information to 3rd parties;

  • Despite reasonable efforts to protect the privacy and security of electronic communications, it may not be possible to completely secure electronic information;

  • Employers and online services may have a legal right to inspect and keep electronic communications that pass through their system; and

  • Electronic communications can be forwarded, intercepted, circulated, stored, or even charged without the knowledge or permission of the participant.

 

When signing this consent form before engaging in theraputic services on the Janeapp and engaging in virtual counselling over Zoom, you accept the risks associated with the use of electronic communications as outlined above.

Sessions and Fees Sessions are in 50 min time slots.

 

The frequency of sessions will depend on time, financial resources, availability, treatment plan, and your personal wishes. Session fees are $155 dollars per hour. Fees are to be paid before completion of a session.

 

If you are registered and approved by the Federal Nations Health Authority (“FNHA”), your practitioner will bill them directly and no money will be due at appointment. If the request of payment to FNHA is not fulfilled, responsibility for the full amount of the session falls upon the client and they must pay the practitioner the full amount before rebooking their next session.

Sliding Scale

Two Feather Wellness understands that accessing affirming and culturally responsive therapy can be difficult to access, especially for youth. I do offer a sliding scale for these situations and the cost will be decided on a case by case basis. In addition, Two Feather Wellness also has one spot available at $10 a session for a LGBTQ+ and/or Indigenous youth aged 14 – 27.

Withdrawing Consent and Other Concerns

Please connect with your counsellor about any questions or comments you may have about your counsellor’s approach, training, experience, or the counseling process at any time. Participation in therapeutic services is voluntary. Client and/or Counsellor have the right to terminate therapeutic services at any time. You may also withdraw this consent or refuse a specific approach to counselling at any time. If you are unhappy or have a concern about our services, we encourage you to discuss this with your counsellor. However, if we are not able to resolve the issues or concerns together, you can contact your counselor’s Licensing Body directly and/or terminate services at any time.
